Sorry, that's a woeful idea. Having the smaller pills later would make people think there was less harm in not taking them and that would lead to an increase in the already ridiculously high proportion of courses of medicine that are not completed and will eventually make all anti-biotics useless.
Would be better to reverse the order and have the large ones last, though that assumes people would read the instructions and the production costs would be justifiable.
Better still is have two colours of the same pil and instruct patients to take 16 red ones followed by 8 blue ones. Much more likely to succeed.
Ooops, I got all constructive there. Sorry.
